Yep, totally depends on what you do day to day I guess. I do the carrier and stroller combo rather than a double buggy. But. I don't do town. Hardly ever go. We live in the country and I've just never been much of a town person. When we do go I either do the carrier/stroller combo or carrier and Jude walks. But I hold his hand - he doesn't hold mine. It's not an option to not hold it. If he has a tantrum and decides he doesn't want to hold it - I'll be one of "those" woman you see dragging their toddler by the arm through the shops to the car  (But town semi-terrifies me with small children! I'm much more relaxed on the farm or at a park etc where Jude can do his own thing and ride his bike or run around  )
Anyway, to answer your original question. Only extra we got was a carseat. One thing I wish I had gotten that you may already have anyway was a baby monitor. Didn't need it with Jude as we were living in town and had a small house but with Willem I wanted one as Jude always wanted to go outside and play in the sandpit etc away from Willem's room where he was sleeping. I still wanted to be able to hear him cry. But never got around to getting one because of the aforementioned not doing town business.  Other things that are handy at the moment but not necessary at all - a runner bike for the older child and we also find the iPod a great distraction for the older child when feeding etc (  ). In the early days also when Jude was getting used to another baby taking up some of his time - we designated one of his soft toys as his "baby" when changing Willem and he would wipe and change his baby at the same time so that he wasn't getting annoyed with the lack of attention. So, if you haven't already got a suitable soft toy or doll you can use, I would suggest one of those too. (Lol, funny that it is mostly all stuff for the elder child I'm suggesting!).

Oh Tyler will hold my hand,but if he decides he doesn't want to hold it,I can't be arsed with having to stop what Im doing just to take him out of the situation,would rather get the job done quickly so I can get home and watch Jeremy Kyle,Dr Phil and Oprah.

Hehe, I hate town anyway so it's a convenient excuse for me. Oh woops, toddler won't hold my hand anymore. Oh well, time to go home, I obviously can't do the errand anymore.

Bahahahaha, and that's where my ability to (pretend to) be blindly oblivious to things comes in handy. I can hold out longer than DP and so he ends up going to the supermarket. Or he just hates the supermarket less than me. Either way, it works for me  .
DP - "Man, we probably need to go to the supermarket sometime eh?"
Me - "Hmmm, what? Oh. Yeah. Maybe. Probably."
DP - "I'll go on Thursday night then. *Sigh*"
Me - Secretly punching the air in my mind because that means another month of me not having to step inside a supermarket.

TOTALLY off topic, but if only I was more like you guys, I'd be rich. With me, it's 'Oh, we've nearly run out of this, ooops, must run to the supermarket (and bring back twice as much as I went for while I'm at it)'. Sigh. Jacob loves the supermarket, and I like food shopping...

Lol Delli! My DH has resigned himself to the fact that grocery shopping is now his job. He is way better at it than me - he spends less as he can stick to the list whereas I always seem to find things we 'need'. Plus he is much quicker. Oh and I'm pregnant, so I'm far too tired to do it in the evenings
He's a bit of a geek though and has set up a spreadsheet in the order of the layout of the supermarket (so he can print out a list) and records the prices each week so he can see how much it will cost
